A thyroglossal duct cyst and surrounding tissues are removed during the Sistrunk procedure. The reasoning behind this procedure is that curing thyroglossal duct cysts will be ineffective unless the epithelium-lined tract (extending from the cyst to the foramen cecum of the tongue) is completely removed, and that dissecting the tract above the hyoid bone will often result in breaking off the duct between the hyoid bone and the foramen cecum. A transverse incision is made across the neck near the hyoid bone, and the skin and platysma muscle are reflected. The cyst is dissected free from the surrounding tissues up to the hyoid bone (usually located beneath the raphe connecting the sternohyoid muscles). The muscles attached to the hyoid bone's center are separated.
